The State Department has now said that all male nonimmigrant visa applicants
between the ages of 16 and 45, regardless of nationality and regardless of where
they apply must fill out and submit to the post a DS-157 in addition to the
usual Nonimmigrant Visa Application (DS-156). [See DOS cable posted on

Individual posts have been given the discretion to require submission of the
form from women too. On January 31, Jerusalem apparently became the first post
to require all applicants between the ages of 16 and 45, of either
gender, to submit the DS-157 form.
