USCIS alert introducing Form I-910, Application for Civil Surgeon Designation and a $615 filing fee. The application will be filed at a USCIS Lockbox and processed at the NBC. Prospective civil surgeons should not file the form and fee prior to 3/11/14. Changes do not affect current civil surgeons.
