SSA “Request for Employer Information” Letter

SSA provides a sample of the letter sent to employers requesting corrections to the Wage and Tax Statement (W-2) form or Corrected Wage and Tax Statement (W-2c) form when there is “no match” between employee names and Social Security numbers and information in SSA records.
