USCIS Update on Revised Form I-693

USCIS announced that it has revised Form I-693, Report of Medical Examination and Vaccination Record, because of changes to the TB component of the CDC’s Technical Instructions for Civil Surgeons. This revised form must be used for any medical examination completed on or after May 1, 2008.
