SSA Revised Information Collection on Form for Requesting Information on Wages Earned
On 1/26/09 SSA published a revised information collection on Form SSA-L725, Letter to Employer Requesting Information About Wages Earned by Beneficiary. SSA uses Form SSA-L725 to determine and verify a beneficiary's wages when SSA has incomplete or questionable wage data. (74 FR 4498, 1/26/09)
